I like to use the capsule endoscope, consists of a small, wireless, pill-sized camera that can be kept in the throat and allows direct visualization of the all the parts involved in speech.
The camera is fed through the patient’s nose and positioned just above the throat. Once the camera is in place, patients can talk. With real-time throat images from the camera,puberphonia boys can learn to control their throat’s effect on breathing.We can also monitor it.
This novel approach that involves visual biofeedback, training in specific breathing techniques, and feedback regarding performance.They can see the mechanical changes in the throat, almost all of these boys have thoughts during talking that are working against wish. Therapeutic laryngoscopy during talking creates an opportunity to address both mechanical issues and performance psychology issues,

Breath of fire in puberphonia treatment.
Buberphonia boys is instructed in inhaling or “sniffing in” through the nose and exhaling gently through pursed lips. Attention is given to abdominal expansion during inhalation and abdominal relaxation during exhalation.
We give feedback, visual imaging techniques are taught as a method of keeping the vocal cords open during rapid breathing. These include visualizing the throat being as wide open during inhalation.
Control breathing include an open jaw and relaxed-tongue position while concentrating on diaphragmatic breathing.
These exercises are initially instituted in breathing, and finally during speaking.
Puberphonia/practice abdominal breathing
Clavicular / intercostal breathing create tension in neck and larynx. This contributes to laryngeal tightness and spasm. Diaphragmatic breathing :results in a relaxed shoulder girdle and abdominal expansion.

Speech is reliant on the powerful air flow that is supplied via our respiratory system. When we breath out,the sound filled air travels up from laryngopharynx to oropharynx, powerful air flow is created in the direction of the air flow to buccal cavity.

The incidence of puberphonia in general population is more than we expect. The most common symptoms include pitch breaks, childish voice,hoarseness, breathiness, difficulty in vocal projection, and visible laryngeal muscle tension.
Puberphonia can be compared with iceberg example, because Puberphonia is an iceberg, iceberg means that only 10% of the problem is on the surface and the other 90% is hidden underwater like deep-rooted psychological & emotional disorder, and by the treatment of this it can be ccured.
Parents commonly neglect this disease, and they think that it will be automatically corrected, but in this, the boy suffers the most because their peers laugh at them, everyone makes fun of them, do mimicry of them, and due to this they feel humiliated.
From their social life, their mind gets disturbed, their confidence gets disturbed, they have low self-esteem, they do not want to go in front of people, they don’t want to sit in a group of people, everyday they feel that there is something missing in them, if they want to say something they stop themselves because they think that if they speak they might be identified as puberphonia, people will laugh at them, others will make fun of me and won’t listen to me like this. The treatment not only simulates the motor system but also incorporates sensory awareness training to help individuals with Puberphonia recognize that their voice is too soft, convincing them that the louder voice is within normal limits, and making them comfortable with their new louder voice.
1. Mouth must be made as a big cavity. It improves vocal loudness by stimulating the muscles of the voice box (larynx) and speech mechanism through a systematic hierarchy of exercises. Focused on a single goal “speak LOUD!” – the treatment improves respiratory, laryngeal and articulatory function to maximize speech intelligibility. The treatment does not train people for shouting or yelling; rather, full mouth uses loudness training to bring the voice to an improved, healthy vocal loudness with no strain.
2. Speak with whole body. When you speak move the hand from abdomen to the head. The sound travel with the hand. You can really feel it.
3. Speak with the movement of soft palate.
4. The ward should come out by the movement of tongue.
5. Flex the head and extend the dead while speaking.
